Proxy Form & Information

A Voting Member entitled to vote at a general meeting may appoint one (1) other Voting Member or the President as their proxy, to attend and vote in their place at a general meeting.

The proxy must be appointed using this form and signed by the Voting Member appointing the proxy.

The proxy is invalid unless the signed proxy form is received by the Association by 6:30pm on the 24 November 2025.
